This publication investigates the impact of postoperative Acute Kidney Injury (AKI) severity on short- and long-term patient outcomes. It is based on a retrospective, single-center cohort study conducted at an urban, academic medical center from January 2015 to May 2020. The study included adult patients undergoing elective, non-cardiac surgery with a postoperative length of stay (LOS) of at least 24 hours.
The study underscores the significant impact of any stage of postoperative AKI on patient outcomes, including increased mortality rates, longer hospital stays, and higher healthcare costs. Even minor AKI events (stage 1) were associated with elevated risks, highlighting the importance of recognizing and managing AKI in postoperative patients. The findings suggest that interventions aimed at preventing or mitigating AKI could potentially improve patient outcomes following surgery.
This article discusses the implementation and evolution of Enhanced Recovery After Surgery (ERAS) protocols at Virginia Commonwealth University Health System (VCU Health). ERAS, initially developed in 1997, aims to improve patient outcomes throughout the surgical journey by incorporating evidence-based practices spanning preoperative, intraoperative, and postoperative phases. VCU Health utilizes data analytics to monitor the effectiveness of ERAS protocols, identify areas for improvement, and track key performance indicators (KPIs) aligned with clinical and business objectives. By analyzing data, VCU Health has identified trends and implemented practice changes to optimize patient care, such as minimizing intraoperative fluid variation to reduce acute kidney injury and standardizing surgical practices to decrease surgical site infections and opioid usage.
The success of ERAS implementation at VCU Health is attributed to the commitment of senior leadership, the availability of comprehensive data, and a culture of continuous improvement. Regular meetings involving department chairs, clinical effectiveness staff, and clinical teams ensure collaboration and facilitate the adoption of new processes. Through data analysis and evidence-based practice changes, VCU Health has achieved significant improvements in patient outcomes, reduced costs, and expanded ERAS protocols across various surgical specialties. The article emphasizes the importance of data-driven decision-making in enhancing accountability, driving cultural change, and ultimately improving patient care.
